Primary Care Transformation (PCT)

We will work to support our member practices to develop a sustainable, resilient and strong foundation of core general practice. Our transformational work will capture and share best practice approaches to improving the efficiency and effectiveness of working at individual practice and locality level.

Through adoption of the ’10 high impact actions’ and other examples of best practice we will support practices to exploit opportunities to embed new ways of working and delivering general practice care, releasing time.

We will work to foster collaboration through formal and informal means between practices informed by member practice appetite. We will develop links and capability to share clinical and back office resource to deliver a more efficient, sustainable model of care.

We will work with partner providers to solidify the links between general practice and community, mental health, social care and voluntary sector practitioners embedding an extended general practice team to enhance integrated working.
